Position: Case Manager

Reports to: Case Manager Coordinator

1. Purpose:

Under The direct supervision of the Case Manager Coordinator, the Case Manager is responsible  for providing case management services to felony offenders committed to the Eastern Ohio Correction  Center. Such services include the coordination of assessments, development and continued monitoring  of a structured treatment plan, and any other activities required in assisting and supporting offenders  in gaining access to needed medical, psychological, social, educational, employment, or other services  essential to meeting basic human needs.

1. Essential Functions and Responsibilities:
2. Administers assessment instruments to offenders and interprets results to determine and/or  identify specific treatment needs.
3. Assists the offender in identifying and developing specific treatment goals and objectives.  3. Meets face-to-face with offenders and discusses progress towards goals and objectives,  treatment services, and/or other areas of identified need. Ensures that individual offenders are  continuously supplied with the resources necessary to achieve goals and objectives.  4. Explores all resources for necessary services and makes referrals where appropriate and  available to community agencies, organizations, and institutions.
4. Acts as an advocate for the offender to ensure that the proper services are being delivered  according to the established treatment plan and to ensure the continuation of such services  until such a time as all goals and objectives identified with the service are successfully achieved.
5. Prepares and maintains treatment plans, reports, progress notes, periodic evaluations, and  other written data or information regarding an offender’s individual treatment. Provides  supervisors and other specified personnel with summary reports, evaluative reports or other  data when requested.
6. Participates in treatment team meetings with other case managers and service providers to  identify offender successes and failures, review cases, review and act upon disciplinary  infractions or other behavioral issues, and make alterations to an offender’s individualized  treatment plan if necessary.
7. Serves as a representative of the facility, displaying compassion, fairness, integrity, and  professionalism in all interaction with other members of the Eastern Ohio Correction Center  community and the general public.

III. Other Duties & Responsibilities:

1. Communicates with probation officers regarding an offender’s progress within the program,  revisions to the offender’s treatment plan, special behavioral, disciplinary, or supervisory  issues, and makes recommendations for the offender’s aftercare or post-release treatment  activities.
2. Assists program facilitators when necessary and/or required in delivery of program services.  3. Provide supportive, individual counseling or crisis intervention services when required or  requested.

4. Assists facility security staff when required or requested.
5. Transports offenders to treatment orientated-functions and appointments within the  community.
6. Collaborates and participates with other staff in designing, developing, and implementing new  treatment programs and offender activities.
7. Participates in relevant professional organizations and attends meetings, conferences,  workshops and seminars sponsored by professional organizations.
8. Participates in facility committee meetings and other facility functions.
9. Performs other incidental related duties as required and assigned.

1. Education and Abilities:

The Case Manager must meet the following minimum educational and experiential requirements:  

1. i) A bachelor’s degree from an accredited college or university in an appropriate field of study (behavioral science, social work, psychology, criminal justice).
2. ii) A valid motor vehicle operator’s license.

OR

1. i) An associate’s degree from an accredited college or university in an appropriate field of study (behavioral science, social work, psychology, criminal justice), PLUS any combination of work-related experience, training, or college credits demonstrated at a level of  achievement to a bachelor’s degree.  
2. ii) A valid motor vehicle operator’s license.

OR

1. i) An associate’s degree from an accredited college or university in a NON-RELATED field, plus any combination of work-related experience, training, or college credits earned in a RELATED field, at a level of achievement of a bachelor’s degree. 
2. ii) A valid motor vehicle operator’s license.
